Job Description
The Care Manager Extender provides care management functions, documentation, referral and linkage, and monitoring/follow-up under direct and indirect supervision. They work with clients, family, friends, and providers to achieve desired outcomes and facilitate provider choice process. The role involves coordinating services, connecting recipients to programs and services, making referrals, and facilitating communication among service providers and the recipient.
Requirements
- High school diploma or equivalent
- Certified Peer Support Specialists
- Community health workers (CHW)
- Individuals who served as Community Navigators
- Parents or guardians of an individual with an I/DD or a TBI or a behavioral health condition
- A person with lived experience with an I/DD or a TBI or a behavioral health condition
- 2 years of paid care management type experience
