Care Manager Extender

The Care Manager Extender provides care management functions, documentation, referral and linkage, and monitoring/follow-up under direct and indirect supervision. They facilitate provider choice process, ensure signed Authorization to Disclose Health Information forms are obtained, and assist the person to obtain the outcomes/skills/symptom reduction that they desire.

Requirements

• High school diploma or equivalent
• Certified Peer Support Specialists, Community health workers (CHW), or individuals with lived experience with an I/DD or a TBI or a behavioral health condition
• 2 years of paid care management type experience with at least 1-year paid experience with population served
