Clinical Associate - Palliative Care

The Clinical Associate is a liaison between our healthcare services and Devoted members, working closely with clinicians to provide high-touch telehealth visits and support care coordination. Responsibilities include preparing members for visits, processing and coordinating palliative and hospice care orders, and coordinating with home health partners and PCPs. The role requires a strong advocate for members, comfortable with care coordination for a Medicare population, and able to ease stress and anxiety of members during difficult times.

Requirements

• MA Certification (highly preferred)
• Fully bilingual (English/Spanish)
• Minimum of 2 years of outpatient experience, working on any of the following healthcare settings: Palliative/ Hospice/ Oncology/Dialysis Centers/ Behavioral Health
