Job Description
The System Navigator position at the HART Hub in Durham, Ontario, is a permanent, part-time (0.5 FTE) position supporting individuals experiencing homelessness, addiction, and/or mental health challenges. This role involves building trusting relationships, coordinating services, and advocating for access to resources. It operates a 24/7 harm-reduction, trauma-informed care model, supported by a core team and community partners, aimed at improving engagement, continuity of care and outcomes for clients.
Requirements
- Bachelor of Social Work, Nursing, or other relevant health discipline.
- 2 years work experience in the Health and/or Social Work field, or an equivalent blend.
- Registration with a regulated professional body (e.g. OCSWSSW, CNO).
- Experience working with marginalized populations.
- Demonstrate understanding of social determinants of health.
- Experience working in a multi-disciplinary team.
- Strong knowledge of local systems (housing, shelters, ODSP/OW, healthcare, harm reduction, addictions services).
