Job Description
Elara Caring is seeking a Registered Nurse Case Manager to join their team. The ideal candidate will have a commitment to compassion and a passion for delivering high-quality care to patients in their preferred home setting. As a Registered Nurse Case Manager, you will contribute to Elara Caring's mission by assessing patient needs, communicating with the clinical team, and teaching hospice philosophy and pain and symptom management.
Requirements
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- Current State License as a Registered Nurse RN
- 1 year of experience in a clinical care setting
- Experience in a hospice or home health environment is preferred
- 50% travel required
- Valid driver’s license and insurance and reliable transportation to perform job tasks
