Bachelor’s Degree in the social service field with two (2) years of experience in care coordination/case management or minimum 4 years of case management and care planning experience in lieu of Bachelor’s Degree

1-5 years of experience in housing navigation or case management with housing experience

2+ years of experience with eligibility and/or referrals, or 3 years in eligibility and/or referrals related field. Associate degree required

1-2 years of experience in care coordination/case management/Community Health work or minimum 2 years of case management/community health work and care planning experience in lieu of Associate’s Degree

High school diploma or GED equivalent, completion of a formal Medical Assistant or Medical Office Assistant program preferred, and one year of experience in referrals or patient services in a medical clinic or healthcare setting

MD/DO/NP/PA w/2-4 yrs exp. BLS/DEA req. California RN/NP/NPF cert. Immunizations/COVID-19 req. Current CA driver's license and insurance (for work hours)

Bachelor's degree in health-related field preferred. 2+ yrs HCIT exp. Equivalent education & experience (clinical setting). Technical knowledge of HIT systems and Microsoft Office Suite

High school diploma or equivalent and Bachelor's degree preferred. 2+ years experience in healthcare, informatics, or similar roles. Clinical setting experience is important

Master’s in Social Work (approved by the CA Board), current Clinical Social Worker license, and experience with integrated behavioral health. Must have a valid driver’s license and proof of vaccinations

High School Diploma or equivalent required, Associates degree or higher preferred, 12 months of work experience in a similar role, Bilingual/Multilingual in English/Farsi/Spanish/Russian/Ukrainian
