We are growing our CalAIM program at Elica!
Requirements
- Perform pre-outreach activities for new clients referred by Managed Care Plans (MCPs)
- Independently manage at least one ECM Referrals EPIC WorkQueues
- Track the status of authorization requests
- Track and resubmit re-authorization requests as required by MCPs
- Coordinate with the CalAIM Quality Assurance Specialist and CalAIM Reimbursement Specialist
- Check eligibility of members in outreach and enrolled programs stage
- Notify an appropriate ECM Care Manager or Community Health Worker if a patient loses coverage
- Provide accurate data entry of referral or authorization information into the electronic medical record
- Attend Team and Managed Care Plan meetings
- Maintain consistent attendance and punctuality
