Job Description
The Case Manager participates as a member of the Foundry Langley team. The Case Manager helps youth and young adults with complex needs by coordinating care, performing follow-ups, and providing service navigation and resource information.
Requirements
- Minimum Experience: Experienced
- Direct service delivery experience in community social services sector
- Excellent interpersonal and conflict resolution skills
- Ability to operate within professional boundaries
- Solution-oriented to meet challenges facing participants
- Knowledge of building resilience and community belonging through the youth drop-in model
- Demonstrated experience in competently assessing the needs of youth
- Strong interpersonal skills
