Physician, Family

A Primary Care Physician provides the full scope of services including diagnosis, treatment, coordination of care, preventive care, and health maintenance. They must be committed to the philosophy, goals, and objectives of the Center and work collaboratively to achieve them. The Physician must model and uphold the mission, vision, and standard of performance within the health center.

Requirements

• Current licensure as an MD to practice in the State of Indiana by the State Board of Medical Examiners.
• Current DEA, CSR Registration
• Board Eligible or Boards Certified in physician specialty
• Insurability (malpractice)
• Current BLS or PALS
• Must be able to meet and maintain current Eskenazi Health credentialing and privileging requirements
