- Maintain caseload of 15-20 AOT members.
- Perform in-person visits and perform key transitional care coordination services.
- Provide assistance with life skills, housing, vocational rehabilitation, and reintegration services.
- Connect members to community support services and outpatient health services.
- Document all encounters and interventions.
