Health Home Plus Care Coordinator

Essen Health Care provides high-quality, compassionate medical care to vulnerable individuals in New York City through its large, rapidly growing multispecialty medical group. They operate under a Population Health model and offer a variety of services, including urgent care, primary care, and specialty services. The Health Home Plus Care Coordinator is responsible for case retention and maintaining a caseload of 20 HH+ members, with eligibility for case adjustment based on DOH recommendations.

Requirements

• Maintain full responsibility for caseload, including Assessments, Care Plans, HML’s, timely documentation
• Conduct home visits and fieldwork
• Ensure follow-up by verifying client participation and providing education
• Document all services in a timely manner
• Attend weekly care management meetings and participate in quality improvement programs
