The Enrollment Specialist is responsible for processing patient enrollments and updates into the lead health home's database, ensuring compliance with regulatory requirements and adhering to departmental and State guidelines.

Requirements

• Researches, verifies, and processes patient enrollments
• Contacts outreach specialists to verify and obtain additional documents
• Communicates with outreach team to walk them through the application and update process
• Assures lead health home compliance with data sources and document accuracy
• Responds to inquiries on enrollment status and update submissions
• Supports department's administrative goals and requirements
