Job Description
Essen Health Care is seeking a highly motivated Outreach Specialist to provide outstanding customer service to prospective Health Home Members, serving as an intermediary between providers and members. The role involves outreach activities, community engagement, patient education, and connecting eligible members with community resources. The company offers a compassionate and accessible medical care in a multispecialty setting.
Requirements
- Achieve daily enrollment targets and provide care support services to members with chronic health conditions.
- Perform outreach activities, especially during house calls, PCP, and Specialty services.
- Educate potential members about Essen Health and the Health Home division.
- Adhere to lead health home's outreach and enrollment standards.
- Use HH internal screening to write attestations.
- Connect eligible members to immediate community resources.
- Provide immediate intervention for urgent needs.
- Complete consent documents for new enrollments.
- Perform field visits to enroll members.
- Complete timely the consenting documents.
