Job Description
The Outreach Specialist will provide outstanding customer service to potential Health Home Members, acting as an intermediary between providers and members, and connecting members to community resources as needed.
Requirements
- Advanced degree including AAS, BA, or BS degree
- Minimum of two years experience in performing community or targeted outreach or two years in healthcare, community health, supportive housing, or other social service case management
- Excellent customer service, organizational, interpersonal, and communication skills
- Ability to multi-task and work under multiple priorities and deadlines in a fast-paced environment
- Proficiency in Office Suite
- Proficient in Spanish and other languages preferred
