Case Manager

This case manager position involves planning, implementing, and coordinating comprehensive, individualized client services for clients in a rehabilitation program. The role also includes psychoeducational instruction, administrative duties, and coordination with agencies and resources. The ideal candidate will be detail oriented, capable of working under pressure, and possess strong client service skills.

Requirements

• Bachelor's Degree from an accredited institution or equivalent combination of education and experience
• Experience with case management
• Knowledge of substance abuse, mental health, health, housing, and community support services
• Experience managing patient services and computer records
• Knowledge of Joint Commission and DHS standards
• Strong interpersonal and analytical skills
• Exceptional customer/client service
• Logically analyze problems and handle multiple priorities
