Job Description
The Case Manager position at a Certified Community Behavioral Health Clinic (CCBHC) provides case management services to clients within the clinic's scope. This role focuses on creating a comprehensive range of mental health and substance use disorder services, including referrals, problem-solving, and advocacy. The ideal candidate will be creative, innovative, and adaptable.
Requirements
- Provide case management services to assigned caseload.
- Develop and monitor individual service plans.
- Assist agency consumers with accessing needed community services.
- Act as an advocate for consumers.
- Documentation of all services in compliance with agency standards.
- Maintain a strong working relationship with therapeutic support services.
- Adhere to on-call cell phone system.
