Job Description
Applewood Village is seeking a compassionate and skilled Primary Care Nurse Practitioner to provide comprehensive primary care services to senior residents. The role involves developing and implementing individualized care plans, promoting wellness, and managing chronic conditions in collaboration with other healthcare professionals.
Requirements
- Master's degree in Nursing from an accredited program.
- Current Nurse Practitioner license in New Jersey.
- Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) preferred.
- Minimum of 5 years of experience in a primary care or geriatrics setting.
- Excellent clinical skills and knowledge of geriatric care.
- Strong communication and interpersonal skills.
- Ability to work collaboratively in a team environment.
- Compassionate and patient-centered approach to care.
