Case Manager, Pathway Home 5

The Case Manager will provide comprehensive reentry services to individuals incarcerated and recently released from NYS DOCCS facilities, including case management, career planning, and job retention support.

Requirements

• High school diploma or equivalent required; Associate's degree preferred
• 1-3 years of relevant experience in human services or workforce development
• Bilingual (English/Spanish) preferred
• Ability to work a flexible schedule and travel between assigned facilities
• Must have initiative, be willing to work in a fast-paced environment, and have excellent communication skills
