New Participant (Patient) Advocate

Habitat Health is looking for a New Participant Advocate to ensure new PACE participants have a smooth transition into the program, with a focus on understanding their needs and expectations, and introducing them to the care team.

Requirements

• Strong organizational and communication skills
• A minimum of one year working with the frail or elderly or case/client management
• Ability to lead with influence
• Proof of valid CA driver's license, personal transportation, good driving record and auto insurance as required by State law
• Bachelor's degree preferred
• Bilingual skills
