Job Description
This temporary, full-time position for a Care Coordinator in Ontario (Marathon) is seeking an experienced healthcare professional. The role involves assessing patient needs, providing connections to community services, and supporting patients with personalized care plans. The role offers opportunities for professional growth and work-life balance within a supportive and collaborative team.
Requirements
- Membership in Good Standing with relevant regulatory bodies (Nurses of Ontario, Physiotherapists of Ontario, etc.)
- 2+ years of recent experience in community health or a related field
- Knowledge of the health care delivery system and community resources
- Excellent interpersonal, communication, assessment, problem-solving and decision-making skills
- Effective time management, prioritization and organizational skills
- Established ability to accurately complete required documentation and forms
- Valid driver’s licence and access to a reliable vehicle
- Proficiency in a Windows environment
- Experience working with diverse patient groups
