Job Description
Evaluates members for case management services, determines level of care coordination/management services, and develops care plans. Travel to inpatient bedside members' homes, providers' offices, hospitals, etc.
Requirements
- RN license - clear and active in the state of NY
- 1+ year of current case management experience
- Managed care experience preferred
