Job Description
HealthCare Support is seeking an RN Field Case Manager to fill an opening with a prestigious Healthcare Organization in Bronx, NY.
Requirements
- Clear and active NY RN license
- Certified Basic Life Support
- Full COVID-19 vaccination
- 1+ year of clinical experience in practice (long-term care setting preferred)
- Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
- Understanding of Geriatrics and Chronic Illness
- Proficient computer skills including the ability to document medical information with written and electronic medical records
- Effective Communication skills and time management
- Ability to work with diverse care teams in a variety of settings
- Access to reliable transportation
- Ability to travel in between facilities
