The ICMS Case Manager provides high quality case management services to clients eligible for permanent housing through the Housing for Health (HFH) program. They conduct initial and quarterly housing/activities of daily living assessments, develop and implement individualized care plans, and provide housing retention and navigation services.
Requirements
- Conduct initial and quarterly housing/activities of daily living assessments
- Develop and implement an individualized care plan
- Write detailed case notes in G.I.R.P. format
- Maintain updated calendar with client visit locations and times
- Develop professional relationships with clients and maintain a case load of 15-20 participants
- Provide housing retention and housing navigation services
- Conduct regular face-to-face home visits and check in meetings with clients
- Ensure that clients are linked to health, mental health, and substance use services as needed
- Assist clients with obtaining employment, and/or establishing benefits, and/or educational opportunities
- Assist clients with maintaining medication and treatment regimens
- Utilize a harm reduction, client focused and strengths based model
- Provide ongoing case management services that will ensure successful permanent housing, decrease social isolation, and prevent relapse risks
- Respond to urgent requests for assistance from clients or landlords
- Participate in all meetings with clinical supervisors, collaborative agencies, and other community meetings as directed
