Job Description
Helpline Youth Counseling (HYC) is seeking a compassionate and dedicated Enhanced Care Management (ECM) Lead Case Manager to coordinate health, behavioral, dental, and social services for high-need Medi-Cal enrollees. The ECM Case Manager's goal is to improve health outcomes, reduce health disparities, and enhance continuity of care.
Requirements
- Bachelor’s degree in Behavioral Sciences or related field
- Minimum one (1) year of experience in community-based services
- U.S. citizenship or permanent residency
- Ability to pass criminal background clearance and TB screening
- Valid CA driver’s license, insurance, and reliable transportation
- Flexibility to work evenings and weekends
- Strong communication, organizational, and documentation skills
