Job Description
The RN Case Manager is responsible for supporting physicians and interdisciplinary teams in enhancing the quality of patient care and managing costs. This role focuses on patient care coordination, discharge planning, and accurate information provision to payers. The ideal candidate will integrate and coordinate Utilization, care coordination, and discharge planning functions.
Requirements
- A.S.N. from an accredited college or university (BSN Preferred)
- Current Alabama licensure
- Three years acute care experience
- Excellent interpersonal communication and negotiation skills
- Strong organizational and time management skills
- Ability to work independently and exercise sound judgment
