Job Description
Hospice Family Care, an independently owned and operated hospice program in Huntsville, Alabama, is seeking a RN Case Manager to provide patient care and support for patients in their homes. The role involves identifying patient needs, prioritizing care, and ensuring patients achieve positive outcomes. Requirements
- Must hold and maintain a current Alabama Nurse License
- Must maintain a current Alabama Driver’s License and automobile liability insurance
- Must be able to lift, position, and/or transfer patients
- Must be able to lift supplies and equipment
- Must have visual acuity and hearing to perform required nursing skills
