Job Description
Homeward is seeking a full-time Care Navigator to work with members and their providers to achieve their best health in rural communities in the Lansing | Ionia | St. Johns area. The role involves proactive outreach, building relationships with members, connecting them with services, and addressing gaps in their care.
Requirements
- Completion of a Medical Assistant program from an accredited program or school
- High school diploma or equivalent
- At least two years of experience in high-touch, patient-facing roles, preferably with seniors
- Passion for delivering care in rural America and ability to persuade members to take actions that support their health
