Job Description
Hope Services is seeking a Community Support Facilitator to provide support and leadership for consumers navigating community activities and services. The role involves assisting consumers with participation, facilitating communication, advocating for consumer rights, and maintaining professional relationships. Hope Services is a leading provider of services to individuals with developmental disabilities and mental health needs.
Requirements
- Provide group and/or individual support to consumers.
- Facilitate opportunities for participation in community activities.
- Advocate for consumers.
- Perform required documentation and record-keeping.
- Perform CPR and First Aid certification if required.
- Strong communication and documentation skills.
- CPR and First Aid certification (Hope can provide training).
