Job Description
Illumination Health + Home is a growing non-profit dedicated to providing targeted services to individuals experiencing homelessness. They are seeking a Housing Navigator to collaborate with property owners and managers to facilitate housing opportunities and provide educational resources. The role involves outreach, assessing needs, coordinating services, and building relationships with landlords and stakeholders.
Requirements
- Maintain caseload of 30-35 clients per hour.
- Enroll families/individuals into homelessness support programs.
- Monitor client progress and collaborate with care team.
- Document all activities and client outcomes.
- Verify client move-in expenses.
