Founded in 1973, National Health Foundation is a nonprofit that works within under-resourced Southern California communities to eliminate barriers to health, address the root causes of poor health, and find solutions that lead to lasting change. The Housing Navigator provides direct assistance to guests experiencing a housing crisis and homelessness through clinical engagement and case management services. This part-time position involves assessing guests' needs, connecting them to resources, creating individualized action plans, and providing stable housing solutions. It requires extensive coordination with multiple internal NHF programs and external entities. Success is measured by guest retention and successful placement in permanent housing.
