Behavioral Health Care Coordinator

Imagine Pediatrics is seeking a Behavioral Health Care Manager (BHCM) to provide case management services to medically complex children in an early childhood setting. The role involves monitoring patients, conducting assessments, and facilitating care plan adjustments based on family needs and professional experience. The position requires experience in behavioral health and a commitment to creating a safe and supportive environment for children and families.

Requirements

• First and foremost, you’re passionate about creating a world where children deserve the best care.
• Provisional licenses (LMSW, PLPC, LAMFT) preferred.
• Minimum 3-5 years of post-graduate experience in health care social work/Case management in behavioral health.
• Experience working with pediatric population and family systems.
• Proficiency in motivational interviewing practices and/or techniques.
• Knowledge of social work including crisis prevention and intervention.
