Job Description
The Case Manager will provide housing case management to Veterans in the VA Grants per Diem Program, ensuring they receive intensive case management services and assistance with obtaining income and housing. The position will involve creating individual housing plans, providing on-going case management, and working with Veterans to achieve personal and economic stability.
Requirements
- High school diploma required, Bachelor's degree in Social Services or related field preferred
- 1 year experience providing direct support services to individuals experiencing homelessness, mental health, substance abuse or other social issues
- Ability to complete CPR/First Aid Training
