Job Description
Lake Sunapee Region VNA & Hospice is seeking a Medical Social Worker (MSW) to provide psychosocial assessment and support to patients in the Home Care and Hospice setting. The MSW will be a resource specialist, aiding patients in navigating illness, long-term care planning, and connecting them to community resources. This role offers competitive rates with flexible schedules.
Requirements
- Master’s degree in social work from an accredited school.
- Minimum of one-year experience in healthcare.
- Excellent communication skills.
- Flu vaccination.
- Valid driver's license
