The Home Health Registered Nurse (RN) provides high-quality patient centered care in the home. This role involves assisting patients with various medical conditions, including surgery and chronic illnesses, in a home environment, managing care plans, monitoring vital signs, and ensuring patient safety. The role requires collaboration with LPNs, physicians, and other healthcare professionals.

Requirements

• Valid license as a Registered Nurse (RN) in the state of employment.
• 2 or more years of nursing experience, preferably with a focus on hospice, palliative care, or related fields.
• Strong clinical skills and knowledge of nursing principles.
• Excellent communication and interpersonal skills.
• Ability to prioritize tasks and manage multiple responsibilities.
• Understanding of regulatory requirements in hospice settings.
• Ability to build rapport with patients.
• Ability to lift and transfer patients.
• Physical dexterity for mobility, physical activities, and manual dexterity.
