Lifelong is a community health organization that removes barriers to health with relentless compassion. We're seeking a Housing Case Manager to assist clients in securing stable housing, address potential issues, and provide supportive services.

Requirements

• Build rapport with clients, identify strengths, and foster growth in self-sufficiency and resiliency
• Provide ongoing support and assistance for formerly chronically unhoused clients
• Advocate for clients with property management and provide resource connections for physical and mental health needs
