Job Description
LifeMoves is seeking a Case Manager ERF to provide outreach services to households experiencing homelessness in San Mateo County's Critical Homeless Encampment Zones. The Case Manager will engage clients, ensure continuity of care, and help navigate complex systems effectively.
Requirements
- Associate's Degree in a related field (or High School Diploma/GED with 2-5 years of equivalent experience)
- Comfortable using basic technology (phone, messaging systems, email, Microsoft and Google Suites, and databases)
- Advocacy skills, ability to identify and connect clients to housing, employment, health, and social services
- Strong analytical skills, ability to research, track, and resolve client needs
- Collaborative relationships with community organizations and service providers
