Clinical Care Coordinator - SAH

We're seeking a Registered or Enrolled Nurse to join our Support at Home (S@H) Clinical Pathways team. In this role, you'll combine clinical expertise with care coordination skills to deliver safe, timely, and client-centred services for older Australians.

Requirements

• Current AHPRA registration (RN or EN)
• Minimum 3 years' experience in aged care, community care, or rehabilitation
• Strong clinical assessment and care coordination skills
• Digital proficiency and ability to work virtually
• Empathetic, proactive, and client-focused approach
