Case Manager

MHA, a nonprofit mental health organization, is seeking a Case Manager to provide support to individuals in substance use and/or mental health recovery with a history of homelessness. The role involves transitioning individuals from homelessness into their own studio apartments, assisting with goal attainment, and supporting access to services like employment and healthcare. The Case Manager will collaborate with a multidisciplinary team and adhere to a trauma-informed, person-centered approach.

Requirements

• Valid driver’s license
• Bachelor’s degree in human services or relevant field
• Six months of experience in individual cases of mental health/substance use and/or housing instability
• Ability to manage time, prioritize tasks, and work independently
• Strong interpersonal skills and compassion
