Case Manager

The Case Manager provides support to individuals in substance use and/or mental health recovery with a history of homelessness, using a trauma-informed, person-centered approach. The role involves coordinating intake documentation, developing individualized action and safety plans, and providing emotional support and access to resources.

Requirements

• Valid driver's license
• Adequate automobile insurance
• Reliable transportation
• Bachelor's degree in human services or relevant field
• Ability to work independently and as part of a team
• Strong organizational, communication, and interpersonal skills
• Must be at least 21 years of age
• Able to obtain/maintain CPR and First Aid certifications
• High school diploma or equivalent
• Minimum of six months of relevant experience
