Requirements
- Doctor of Optometry (OD) Degree from an accredited institution
- License in good standing to practice Optometry in the state
- National Provider Identification (NPI) required
- TPA License required
- Ability to communicate effectively and utilize equipment
- Understanding of patient vision and health care needs
