The Medical Coordinator will provide post-hospital care to guest quality of life in a Recuperative Care Program at National Health Foundation in Glendale, California. The role involves assessment, discharge planning, medical monitoring, coordinating appointments, and collaborating with Intake and other team members. Strong bedside manners and teamwork are essential. This position requires flexibility to manage a caseload and track guest information.
