Job Description
The Medical Coordinator/Licensed Vocational Nurse position at National Health Foundation involves medical coordination and education for guest quality of life in the Recuperative Care Program. They will assess patient needs, maintain their care plan, perform medical and wellness checks, and provide advocacy on behalf of the guest. The role requires flexibility to meet patient needs and maintains high standards of patient care.
Requirements
- Current LVN License or Bachelor's degree in Nursing
- Current American Heart Association BLS (CPR/First Aid/AED)
- Bilingual in Spanish (preferred)
- Reliable transportation and auto insurance
