As the Resident Advocacy Director (RAD) at our Reserve at Mills Farm community, you will serve as a key support in resident care, assisting with clinical coordination, family communication, and program implementation to ensure continuity and quality of services.

Requirements

• Active Licensed Independent Social Worker-Clinical Practice (LISW-CP) or Licensed Clinical Social Worker (LCSW) credentials in North Carolina
• Care coordination background, case management experience, or experience as a hospice social worker
• Experience delivering behavioral health interventions and psychotherapy
