Diabetes Educator - Thunder Bay Site

We are seeking a contract 1 FTE Registered Dietitian or Registered Nurse to provide individual and group diabetes education programs in Thunder Bay. The position has a competitive salary and option to join Health Care of Ontario Pension Plan.

Requirements

• Registered Dietitian or Registered Nurse
• 3 years of experience in Diabetes Care of adults living with diabetes or newly diagnosed
• Certified Diabetes Educator designation is an asset
• Experience in supporting individuals with complex chronic diseases with self-management of diabetes required
• Excellent interpersonal, communication, group facilitation and teaching skills
• Ability to work both independently and collaboratively with an interprofessional team
• Knowledge and proficiency in MS Office (MS Excel/MS Word) and electronic medical record system experience
• Valid driver’s license, and ability to travel to Armstrong and Kakabeka Site with a company vehicle may be required
