Job Description
OakBend Medical Center is seeking a full-time Case Manager to assess, plan, implement, coordinate, and evaluate healthcare needs for patients. The role involves collaborating with patients, families, payers, and healthcare team to maximize efficiency and quality outcomes. Requirements
- Current licensure to practice as licensed vocational nurse, a registered nurse or masters prepared social worker in the State of Texas
- Three to five years' experience in nursing or medical social work
- Prior experience in case management and/or discharge planning
- Valid drivers’ license and reliable transportation
