Job Description
This part-time Case Manager position at OakBend Medical Center assesses patients' health needs, plans and implements wellness and sickness services, and coordinates with healthcare teams. The role focuses on efficient resource utilization and quality cost-effective outcomes, with a requirement for self-directed work.
Requirements
- Current licensure to practice as a licensed vocational nurse, registered nurse, or master-prepared social worker in the State of Texas.
- Three to five years' experience in nursing or medical social work.
- Prior experience in case management and/or discharge planning is a plus.
- Valid driver's license and reliable transportation required.
