Behavioral Health Care Manager (Social Worker)

The Behavioral Health Care Manager (Social Worker) will provide behavioral interventions, psychosocial assessments, and patient services as part of the Collaborative Care Model. The role will involve counseling, patient education, and coordination of care with medical providers and other mental health professionals.

Requirements

• Master's Social Work (MSW) degree
• Certificate of Clinical Social Work Associate (CSWA) or Licensed Clinical Social Worker (LCSW)
• 3 years' experience in a similar position
