Psych Nurse Practitioner (Outpatient) Community Health –South Shore

Great opportunity for a psychiatric nurse practitioner to join a growing team working in a community health environment providing care to the under-served population. The practitioner will work with a team of psychiatrist, Primary Care Physicians and Social Workers to provide comprehensive and appropriate continuum of care.

Requirements

• Performing psychiatric diagnostic assessments for all referred patients
• Prescribe medications and provide medication management
• Assess, plan, document and evaluate patient needs and treatment
• Provide consultation for primary care providers regarding patient treatment
• Provide intervention and referral for patients when appropriate
• Provide counseling, crisis intervention and advocacy to all patients and their families
• Psycho education around such issues as high-risk behavior, mental health, addictions, and domestic violence
