Pacific Health Group is seeking a Street Outreach Advisor to work in the Imperial County community, connecting individuals with care and support services through in-person outreach and partnerships with community-based organizations.

Requirements

• Experience in community outreach, public health, social services, healthcare navigation, or similar field-based work.
• Comfort working independently in a fast-paced, community-facing environment.
• Ability to manage outreach activity, relationships, and follow-through across multiple partners.
