We are seeking a Street Outreach Advisor to work directly in the community to connect individuals with care and support services that improve health and stability. The ideal candidate will have experience in community outreach, public health, social services, or similar field-based work.
Requirements
- Experience in community outreach, public health, social services, healthcare navigation, or similar field-based work
- Comfort working independently in a fast-paced, community-facing environment
- Ability to manage outreach activity, relationships, and follow-through across multiple partners
- Valid driver’s license and ability to travel locally
